- Open to parents of children ages 0-14 years with Phenylketonuria (PKU).
- Open to US residents only. One survey participant per household.
- All survey responses are confidential and are reported only in aggregate.
- Your feedback will help drive improvements to the patient experience.
- The online survey is expected to take about 20 minutes and can be completed from any computer or mobile device.
- If you qualify for and complete the survey, you will receive a $75 incentive for your participation.
- Incentives will be issued within 2 weeks in the form of an Amazon e-giftcard.
